* ** The work includes:*
* + Providing care in the patient's home setting.

+ 5-8 patient visits per day with a mix of home visits, and potentially video visits for established patients.

+ Regular interdisciplinary meetings to prioritize patient goals

+ Collaborating with patient PCP to develop care plan

** Primary Responsibilities*
* + Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues, in collaboration with patient's Primary Care Physician

+ Effectively identifies, evaluates, and addresses disease prevention and health promotion issues while administering quality patient care.

+ Maintains accurate medical records and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations.

+ Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.

* + Advanced Nurse Practitioner Certification in Primary Care, Geriatric, Adult, with unrestricted licensure in the state of NY at time of employment

+ Current and unrestricted DEA certificate required or ability to obtain
