Senior Doc is expanding our Inpatient Palliative Care team and is looking for a Nurse Practitioner or Physician Assistant with a passion for caring for patients with serious or chronic illness.

We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions. This position supports inpatient palliative care, community-based care, and home visits, with flexibility to practice across settings based on patient needs and program demand.
Key Responsibilities- Provide comprehensive palliative care consultations across inpatient, community, and home-based settings.
- Develop and manage inpatient consultation emphasizing comfort, symptom control, and patient goals.
- Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams.
- Serve as a clinical leader within the interdisciplinary team.
- Educate patients and families on disease progression, treatment options, and goals of care.
- Demonstrate strong time management while managing a diverse patient caseload.
- Participate in interdisciplinary meetings, case conferences, and care planning.
- Active California Nurse Practitioner or Physician Assistant license.
- Strong interdisciplinary team leadership skills.
- Excellent time management and organizational abilities.
- Comfortable with home visits and community driving.
- Strong communication skills with patients, families, and care teams.
- Ability to work independently while coordinating care across settings.
- Credentialed at either Orange County, LA County, Long Beach, Temecula Valley, Riverside, San Bernardino, San Diego County area hospitals.
- Flexible, provider-centered work environment.

Hourly range for posted region
$85 - $120 USD
