Cadence Solutions, Inc. is seeking a Clinical Navigator (Licensed Practical Nurse) to provide virtual chronic care support for Medicare patients. The position is fully remote with standard working hours between 8:00 AM to 6:00 PM Pacific time.
The ideal candidate will have an active LPN license, 5 years of experience, and excellent interpersonal communication skills to build patient rapport via telephone.
