Overview

Pine Park Health is a value-based primary care practice delivering care to seniors in their homes and in senior living communities. Our model emphasizes prevention, screening, chronic condition management, lab work, and diagnostic testing, with weekly community visits and close collaboration with facility staff. We aim to minimize unnecessary urgent care and avoid hospitalizations by offering same/next-day appointments, improving access and outcomes for residents.

We are hiring Nurse Practitioners (Full-Time) to support primary care for seniors in the East Bay area.

Responsibilities
  • Provide comprehensive onsite primary care to seniors in assisted living and senior living communities.
  • Develop and execute personalized, patient-centered care plans that address the whole person.
  • Partner with the Medical Director to refine clinical policies, protocols, and guidelines as the practice evolves.
  • Deliver monthly Chronic Care Management (CCM) visits and oversee CCM documentation.
  • Collaborate with senior living leadership and care teams to optimize care plan execution and patient outcomes.
Qualifications
  • Clinical credentials: NP certification with a genuine interest in geriatric medicine (prior geriatric experience preferred but not required).
