Value Based Care LPN for DMC at Home Program
DMC Primary Care seeks an experienced Licensed Practical Nurse (LPN) to join our DMC At-Home Program as a Value Based Care/At-Home Program Nurse (LPN). The DMC At-Home team provides wrap‑around care to a population of DMC patients who are not able to visit our offices in person.
As a Value Based Care Nurse, you will serve as a key member of the At-Home Program care team, supporting the Advanced Practice Providers (APPs) who deliver in‑home medical services through telephonic care coordination and patient management. You will play an integral role in transitions of care by contacting patients following hospital discharge, reviewing discharge instructions, performing medication reconciliation, updating electronic medical records, and facilitating communication between patients, providers, caregivers, and care management teams.
You will also triage patients who call with acute symptoms or clinical concerns, perform a nursing assessment, and determine the appropriate disposition to ensure patients receive the right care at the right time and in the right setting, including self‑management guidance, urgent provider evaluation, emergency department referral, or activation of emergency services when indicated.
Through proactive care management and chronic disease support, the Value‑Based Care Nurse helps patients navigate their healthcare needs, promotes adherence to treatment plans, and addresses barriers to care. The overarching goal is to improve health outcomes and quality of life for patients with chronic and complex conditions, reduce avoidable emergency department visits and hospitalizations, and support value‑based care initiatives that promote high quality, patient‑centered care.

The successful candidate will possess high‑level organizational skills and the ability to multi‑task, work under pressure, while presenting a calm, professional manner and positive attitude.
Location & HoursThis is a full‑time position, Monday through Friday, located in our DMC At‑Home office in Bedford, NH.
Education, Experience, License and Certification RequirementsEducation & Training:
Completion of an accredited LPN program is required.
Experience:
1-3 years of outpatient primary care is required; care management experience is preferred.
Licensure &
Certifications:
New Hampshire State Licensure as a Licensed Practical Nurse is required; BLS Certification is required.
