Title: Registered Nurse – RN Care Manager
Job Type: Permanent / Direct Hire
Setting: Non‑Profit / LTSS Program / Social Work / Geriatrics
Location: Blue Ash, OH (Hybrid role, 50/50 field and home office split)
Schedule: Full-time, Mon – Fri, 8a – 4:30p, can schedule your own patients and no on call!
Pay: $32‑35/hr based on experience plus superior benefits plan (pto, paid holidays, HSA, medical, dental, vision, pension and more)
ResponsibilitiesThe Registered Nurse will provide care management services to a Medicaid program that helps older adults with disabilities receive care and support in their homes, instead of a nursing home. Duties to include: provide intake, assessment, and care management services to clients, which includes ongoing assessment for program eligibility, monitoring all aspects of care and client services (social and medical), and ensuring needs of clients and caregivers are met through a collaborative process.
Requirements- OH licensed Registered Nurse in good standing
- At least one year of experience working with geriatric persons with disabilities/chronic conditions and LTSS
- Prior care management / case management experience and electronic documentation preferred
- Valid driver’s license with good record and reliable transportation
