Summary
The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Home Based Primary Care (HBPC) within the Geriatric Extended Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides direct Veteran care and care coordination as it pertains to Veterans and their care within the HBPC Program.
Duties- The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.
- Core functions are outlined in the clinical privileges that correspond with this position.
- Major duties include, but are not limited to: provide advanced clinical practice, consultation, and management.
- Utilizes interventions that maximize health capabilities and reduce unnecessary urgent care and emergency room visits per Veteran's goals of care.
- Independently manages a panel of patients with acute and chronic health conditions, emphasizing health promotion and disease prevention using evidence-based practice.
- Makes home visits to Veterans in the program.
- Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Demonstrates leadership and works collaboratively with the interdisciplinary team.
- Orders and interprets labs, tests, and diagnostic studies.
- Supports colleagues and other nurses through HBPC knowledge sharing to provide safe, quality nursing care.
- Evaluates outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations.
- Responsible for documented HBPC outcomes at the program or service level.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Initiates referrals and consults to specialty services.
- Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
- Prescriptive authority for a panel of patients or program as indicated by their position and state law.

- Work schedule:
Monday through Friday, 8:00 am – 4:30 pm; telework available. - This is not a virtual position; relocation/recruitment incentives not authorized.
- Permanent Change of Station (PCS) not authorized.
- Licensure: one full and unrestricted license from any U.S. state or territory.
