The day-to-day clinical environment involves collaborating with on-site nursing staff and attending physicians to ensure seamless, high-quality care through virtual platforms. We are seeking an autonomous and compassionate clinician with a strong background in geriatrics, adult, or family care. The ideal candidate is comfortable managing complex chronic conditions and acute changes in patient status from a remote setting. A collaborative spirit and excellent communication skills are essential for thriving within an interdisciplinary team dedicated to patient-centered care.

Key Responsibilities
- Provide telehealth care to long-term care residents in skilled nursing facilities
- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment
- Conduct advanced care planning and manage chronic disease conditions
- Prescribe medications including controlled substances in accordance with Kentucky and Ohio law
- Communicate with nursing home staff and collaborate with attending physicians on care plans ## Qualifications
- Current, unrestricted Nurse Practitioner license in Kentucky and Ohio (required) - National board certification (AANP or ANCC) - Active DEA licensure
- Clinical background in adult, family, or geriatric specialties
- Telehealth/virtual care experience, multi-state licensure preferred
- Experience in long-term care or skilled nursing facility setting preferred
