- Location: 100% Remote (Must reside in Metro Chicago) with up to 60% local travel (2 days/week)

- Compensation: $80,000 - $90,000 base salary + up to $1,000/month bonus potential

We are seeking an experienced, bilingual (Spanish/English)
RN Medical Case Manager to support injured workers throughout Metro Chicago. In this role, you will serve as a vital clinical advocate and care coordinator, working closely with injured workers, physicians, employers, and attorneys to ensure patients receive appropriate, high-quality, and cost-effective treatment plans that support their recovery and return-to-work journey.
This role is ideal for an autonomous RN with strong clinical assessment skills and a background in orthopedics, rehabilitation, neurology, case management, or utilization management.
Key Responsibilities- Care Coordination: Provide both telephonic and in-person medical case management services, assessing, planning, implementing, and evaluating customized patient treatment plans.
- On-the-Ground Advocacy: Travel locally up to 60% of the time to attend physician appointments, provider visits, and conduct initial patient assessments or home evaluations.
- Resource Management: Coordinate essential post-injury services, including durable medical equipment (DME), home health care, and specialized nursing care.
- Stakeholder Communication: Act as the central hub of communication, monitoring patient progress and delivering clear updates to families, medical providers, employers, and legal stakeholders.
- Documentation: Maintain precise, timely clinical documentation and case records to track recovery and return-to-work milestones.
- Language:
Must be fully bilingual in Spanish and English (verbal and written). - Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Illinois.
- Education: Graduate of an accredited School of Nursing (BSN preferred).

Experience:
1–3 years of clinical experience in Orthopedics, Neurology, Rehabilitation, Trauma, Occupational Health, Utilization Management, or Acute Care. - Travel & Transportation: Valid driver’s license and highly reliable transportation capable of handling Chicago winter weather driving conditions. Willingness to travel up to 60% locally (up to 2 hours into the city/metropolitan area).
- Tech Savvy: Proficiency with Microsoft Office Suite (Outlook, Word, Excel) and comfort navigating digital charting systems.
- Experience as a Medical Case Manager, Nurse Case Manager, or Workers' Compensation Case Manager.
- Prior knowledge of Workers' Compensation guidelines and regulations.
- Certified Case Manager (CCM) designation (or willingness to obtain it within 18 months of hire—fully reimbursed!).
