POSITION OVERVIEW- Hybrid RN Case Manager role focused on coordinating care for aging adults living independently at home
- Community-based nursing opportunity centered around care planning, continuity of care, and service coordination
- No bedside nursing responsibilities required
- Hybrid structure combining community patient visits with remote follow-up and care coordination work
- Mission-driven senior care environment with strong interdisciplinary collaboration
- Two remote workdays weekly
- No bedside nursing demands
- No nights, weekends, or call responsibilities

- Monday-Friday schedule
- 8:00 AM-4:00 PM hours
- Three community-based days weekly
- Two remote workdays weekly
- No nights
- No weekends
- No call responsibilities
- Base salary: $84,000-$108,000 annually based on experience

Requirements- Active Massachusetts RN license REQUIRED
- Minimum 2 years RN experience REQUIRED
- Home care experience preferred
- Strong communication and organizational skills REQUIRED
- Comfortable working independently within community settings REQUIRED
- Team-based care approach REQUIRED
- Comfortable with hybrid remote/community workflow REQUIRED
