Title: RN Hospice - Evening FT
Status: Regular, Full-Time
Location: Manahawkin, New Jersey

Position OverviewA leading home-based healthcare organization is seeking a compassionate Registered Nurse to provide hospice care to patients in their homes throughout Ocean County, New Jersey. This role offers the opportunity to deliver individualized, patient-centered care in a home setting while collaborating with an interdisciplinary healthcare team.
Schedule- Full-Time (40 hours per week)
- Evening Shift: 3:00 PM - 11:00 PM
- Every 4th weekend is required
- Holiday rotation: 2 of 6 holidays
- No on-call responsibilities
- Deliver skilled nursing care to hospice patients in their homes.
- Complete comprehensive patient assessments and ongoing evaluations.
- Develop, implement, and update individualized plans of care in collaboration with physicians and the care team.
- Ensure care delivery aligns with physician orders, regulatory requirements, and professional nursing standards.
- Perform medication reconciliation and monitor patient response to treatment plans.
- Provide education and support to patients, families, and caregivers regarding disease management, symptom control, and hospice care goals.
- Document assessments, interventions, and clinical decisions accurately and within established timelines.
- Coordinate care among physicians, case managers, therapists, home health aides, and other healthcare professionals.
- Monitor authorization requirements and maintain compliance with payer guidelines.
- Supervise home health aide services and complete required supervisory visits.
- Communicate changes in patient condition promptly and participate in care transitions.
- Maintain productivity expectations and support quality patient outcomes.
- Participate in ongoing professional development and incorporate current best practices into patient care.
- Associate Degree in Nursing or Nursing Diploma.
- Minimum of six (6) months of Registered Nurse experience or one (1) year of LPN experience.
- Experience in home health, hospice, or community-based nursing preferred.
- Basic computer proficiency and documentation skills.
- Active New Jersey Registered Nurse (RN) License.
- Current AHA Basic Life Support (BLS) for Healthcare Providers certification.
- Bachelor of Science in Nursing (BSN).
- Prior hospice or home care experience.
- Strong care coordination and case management experience.
