As a Registered Nurse Case Manager (RNCM), you will provide one-on-one patient care, coordinate care, and deliver home-based nursing care as directed by an attending physician. You will work at the top of your license and be part of an interdisciplinary team focused on compassionate, quality care and positive outcomes for patients and their families.

How You’ll WorkYou will assume ultimate accountability and leadership for patient assessment, plan of care, and clinical care under physician direction. You will educate patients and families, ensure patient safety, work autonomously, and manage time efficiently. All duties remain within the applicable scope of practice.
Major Areas of Responsibility- Plan of Care:
Observe and monitor patient conditions, perform OASIS assessments to develop individualized care plans and adjust as needed. - Minimize Patient Risk:
Administer medication as prescribed and help reduce readmissions by prioritizing visits for high-risk patients. - Supervision:
Oversee and supervise total care provided by nurse aides and LPNs. - Continuity of Care:
Manage multidisciplinary care while promoting continuity with admissions, transfers, and discharges. - Family Educator/Advocate:
Educate patient and family on disease processes using teach-back methods to ensure understanding. - Policies:
Complete all clinical documentation following agency protocol and Medicare/Federal guidelines; collaborate with the interdisciplinary team and physicians. - Rules and Regulations:
Understand and follow agency policies, procedures, rules, and regulations; communicate schedule/availability changes to schedulers or supervisors. - Operations:
Attend in-service training and mandatory agency meetings.
- Compassionate communicator with a positive attitude.
- Patience when working with patients, families, physicians, and coworkers.
- Attention to detail, observant, and follows directions.
- Registered Nurse with current license in the state of employment.
- Minimum one-year experience as an RN in an acute care setting.
- Home care experience preferred.
- Valid driver's license and auto insurance in your name as a driver.
- Capable of all physical demands.
