Nurse Practitioner-Palliative Medicine Full Time-Bilingual a plus!
Salary: $108k-$135,00 based on years of experience
Seeking a diligent and compassionate Nurse Practitioner for our Palliative Medicine Inpatient team. The NP will work with our Palliative Medical Partners group providing consultation and follow-up visits for patients within our community hospital and specialty hospital partners in New Jersey. This is a full time position, where as the Nurse Practitioner will collaborate with patients, families, and their primary care and consulting physicians to provide excellent patient and family centered care.
The NP will develop and maintain internal and external relationships, while meeting the needs of our patients and their families. Master’s degree in nursing;
License/Certification: RN, APN license in NJ; experience in palliative care, oncology, hospice or geriatric care is preferred but not a requirement.
- Provides a health assessment appropriate to the visit being performed, as well as diagnose and treat patients who require management of symptoms and goals of care conversations.
- Collaborates with patients, families, and the attending physicians and other specialists, to provide excellent patient and family centered care.
- Develop and maintain internal and external relationships, while working with our support staff to meet the needs of both internal and external partners.
- Work alongside our Care Navigators team members to provide coordinated care across all of our Samaritan's family of programs for all patients.
- Provide continuity of care by working with Navigators and Complex Care Coordinator to transfer inpatient patients to our outpatient pall med team.
- Full time M-F (40/week) with after hours call rotation (telephone calls no visits)
- There is an on-call rotation schedule and weekend rotation required with comp day included.
- Prior experience preferred in palliative care, oncology, heart failure, geriatric care, and/or hospice
- RN, APN license in NJ
- DEA and CDS license will be required
- Consultations in our Partner hospital settings
