Family Nurse Practitioner

Family Nurse Practitioner (Women's Health & Primary Care) – Buffalo, New York

We're seeking a compassionate and clinically strong Family Nurse Practitioner to join a mission-driven healthcare organization focused on improving outcomes for underserved communities.

In this role, you'll provide a blend of women's health and primary care services, including:

• Well-woman exams, preventive screenings, and reproductive health services
• Pregnancy-related care and patient education
• Management of chronic conditions such as diabetes, hypertension, and asthma
• Acute care visits and comprehensive health assessments
• Care coordination with specialists and interdisciplinary teams
• In-person, home-based, and telehealth patient visits

• NYS FNP license and national NP certification
• 2-3+ years of NP experience in Women's Health and/or Primary Care
• Strong clinical judgment and patient-centered approach
• Experience working with diverse or underserved populations preferred
• Bilingual candidates encouraged to apply
