As a key member of our team, you will deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

As a Nurse Practitioner or Physician Assistant with a background in Geriatrics or Internal Medicine, you will provide comprehensive preventative care to patients in skilled nursing facilities. This role combines on-site visits with remote work, allowing for flexibility and autonomy in your schedule. You will work closely with our dedicated scribe team to ensure efficient documentation and focus on building strong relationships with our facilities, patients, and their families.

• Provide high-quality preventative care to residents in skilled nursing facilities
• Conduct on-site visits and remote work from home, supported by a dedicated scribe team
• Collaborate with facilities to develop and implement patient-centered care plans
• Build strong relationships with patients, families, and facility staff
• Maintain accurate and timely documentation

• Active NP or PA license in the state of practice
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills

• Competitive salary range: $83,000 - $85,000 annually
