Tru Health is seeking an Advanced Practice Provider, either a Nurse Practitioner (NP) or Physician Assistant (PA), with experience in primary care or internal medicine and a preference for geriatrics focus. This part‑time role involves providing coordinated health care to long‑term care residents enrolled in special Medicare Advantage plans. Position

Summary

As an advance practice provider, you will exercise independent judgment to treat patients with acute and chronic conditions, consulting with a supervising physician as appropriate. You will work closely with facility partners and health plan leadership to implement our national model of care in compliance with all federal and state regulations.

Responsibilities
  • Assess patients’ medical and mental health needs and provide a plan of care that proactively manages their conditions and addresses barriers to care.
  • Ensure compliance with local, state, and federal agencies related to clinical services you provide.
  • Prescribe medications and order lab work, diagnostic procedures, and consultations.
  • Monitor patients’ compliance and response to treatment and modify those plans as necessary.
  • Work with RN case managers as part of an integrated care team.
Qualifications & Credentials
  • FNP, AGNP, AHACNP or PA license required.
  • Degree from an accredited APRN or PA program.
  • At least 3 years of experience in clinical nursing or rehabilitation with geriatric populations.
  • Experience with Electronic Health Records.
  • Working knowledge of Microsoft applications (Word, Outlook, Excel).
Supervisory Responsibilities
  • May be required to provide training and advice to facility staff.
