Nurse Practitioner

Role Description

This is a full-time hybrid role for a Nurse Practitioner, based in Port St. Lucie, FL, with some flexibility for remote work. The Nurse Practitioner will assess, diagnose, and treat patients in Skilled Nursing and Assisted Living environments. Responsibilities include developing and managing patient care plans, collaborating with interdisciplinary teams, ensuring compliance with medical standards and regulations, patient education, charting, and ongoing communication with caregivers and families to ensure comprehensive care.

• Clinical care skills, including patient assessment, diagnosis, and treatment planning
• Proficiency in electronic health record (EHR) systems for accurate documentation and charting
• Strong communication and interpersonal skills for collaborating with interdisciplinary teams, patients, and families
• Knowledge of medical standards, compliance regulations, and post-acute care workflows
• Ability to work independently with flexibility, managing both onsite and remote responsibilities
• Current Nurse Practitioner license in the state of Florida
• Experience in Skilled Nursing or Assisted Living environments is a plus
• Master’s degree in Nursing or related field
