Our field case managers serve as vital members of the care team, providing expert medical case management services to injured workers. You’ll coordinate care, collaborate with treating providers, and guide injured workers through the recovery process—always with the backing of a seasoned and responsive support team.
Who We’re Looking ForWe are seeking
experienced field case management professionals
who:
- Are passionate about patient advocacy and improving outcomes
- Thrive in a collaborative, supportive environment
Have strong communication and clinical assessment skills - Appreciate a role with autonomy, flexibility, and purpose
- Registered Nurse (RN) with active California licensure
- 2+ years of catastrophic injury/illness case management or catastrophic rehab experience, including onsite case management
- Workers’ Compensation experience required
- Must reside in the greater East Bay area for onsite appointments
- CCM certification (or willingness to obtain within 2 years)
- Valid driver’s license
- The ability to work independently with great autonomy
- Experience managing catastrophic cases such as acquired brain injury, spinal cord injury, multiple trauma (including amputation), and burns preferred
