The Population Health Nurse (RN) is a contract-based role with hybrid working conditions, primarily based in El Segundo, CA, with flexibility for some remote work. The role focuses on supporting high-risk Medicaid populations by coordinating and providing clinical services. Responsibilities include streamlining patients' access to screenings, managing patient outreach and follow-up, analyzing results, ensuring compliance with care protocols, and leveraging AI-driven tools to deliver efficient care.
The nurse will work collaboratively with physicians, healthcare organizations, and interdisciplinary teams to address care disparities effectively.
- Active Registered Nurse (RN) license in good standing.
- Proficiency in patient care coordination, outreach, and case management
- Strong data review, analysis, and documentation abilities, complemented by attention to detail
- Experience with electronic health records (EHR) and AI-powered healthcare platforms preferred
- Familiarity with Medicaid-associated care gaps and HEDIS/UDS quality measures is a plus
- Excellent written and verbal communication skills, with the ability to educate and advocate for patients
- Adaptability to hybrid work environments, including both on-site and remote responsibilities
- Bachelor's degree in nursing or related clinical field; advanced certifications are a plus
