Community Health Nurse

The Delaware County Office of Services for the Aging (COSA) is seeking a motivated and compassionate nurse to join our team as a Community Health Nurse. In this role, you will make a meaningful impact in the lives of older adults while working in a supportive and collaborative environment.

• Provide clinical consultation to care managers on client cases.
• Conduct in‑home nursing assessments and evaluations of older adults in the community.
• Deliver training on relevant health and medical topics to older adults, staff, and community partners as needed.
• Manage a blend of office‑based responsibilities and fieldwork.

• Full‑time employment.
• Work hours are 8:30 AM to 4:30 PM, Monday – Friday, with a 30‑minute lunch.
• Telework: may have opportunity to work from home part‑time after six months, depending on operational needs. Telework requires a securely configured high‑speed internet connection and work from an approved location inside Pennsylvania. If unable to telework, option to report to headquarters office in Eddystone. The ability to telework is subject to change at any time.
• Salary: in some cases, the starting salary may be non‑negotiable.

• Two years of community health nursing experience and a bachelor’s degree in nursing, including or supplemented by the required number of credit hours of community health nursing education accredited by the National League for Nursing.
• A master’s degree in nursing or public health accredited by the National League for Nursing or the American Public Health Association may substitute for one year of community nursing experience.
• Any equivalent combination of experience and training.
• Possession of a current license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing, or possession of a non‑renewable temporary practice permit issued by the Pennsylvania State Board of Nursing.
• Employees with nonrenewable temporary practice permits must obtain licensure as a Registered Nurse within one (1) year.
• Must meet the PA residency requirement.
• Must be able to perform essential job functions.

• This position falls under the provisions of the Older Adult Protective Services Act.
• Conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to PA State Police and FBI clearance.

• 711 for hearing and speech disabilities or other individuals.
