Each assessment visit typically lasts about 45 minutes, ranging from annual health checks to post-discharge evaluations, giving you the chance to make a positive impact on patient care in a home-based environment.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Conduct structured annual home health risk assessments for Medicare Advantage participants
• Review and evaluate patients current and previous medical histories, conditions, and medication usage
• Educate patients about their medical issues and prescribed medications
• Identify gaps in care and provide actionable recommendations for resolution
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in the state of NC
• Current BLS (CPR) certification
• Board Certified through AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience (preferred)
• MSN from an accredited nursing program
