Carease Health - OH is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner with our team, you will have the opportunity to work in a hybrid environment, combining on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

• Provide high-quality preventative care to residents in skilled nursing facilities
• Conduct on-site visits and remote work from home
• Collaborate with facility staff to develop and implement patient care plans
• Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to improve patient outcomes
• Maintain accurate and timely documentation using a dedicated scribe team
• Build strong relationships with facility staff and patients
• Participate in quality improvement initiatives to enhance patient care standards

• Active NP or PA license in the state of Ohio
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
• Ability to work independently and as part of a team
• Strong patient-centered approach
