As a Nurse Practitioner focused on home health, youll support quality care for patients residing in Concord, Kannapolis, Locust, and the surrounding areas. • Flexible shifts available seven days a week
• Create your own working schedule
• Minimum hourly commitment on a weekly or bi-weekly basis

• Perform thorough annual health risk assessments for Medicare Advantage participants in their homes
• Evaluate patients medical histories, current health conditions, and medication usage
• Deliver patient education regarding medical issues and medications to support ongoing health
• Identify care gaps and recommend strategies for resolution
• No requirement for developing treatment plans or prescribing medications

• Active Nurse Practitioner license in North Carolina
• Current Basic Life Support (BLS/CPR) certification
• Board Certification through AANP or ANCC
• A minimum of 6 months current nurse practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited program
