Registered Nurse Primary Care; HBPC Outpatient

Duties

Duties of the position include:

• Executing position responsibilities that demonstrate leadership, experience, and creative approaches to managing complex Home based Primary Care (HBPC).
• Providing direct Veteran care and care coordination within the HBPC Program.
• Performing case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
• Planning and delivering technically competent care in the homes of Veterans with complex needs, including health promotion, disease prevention, geriatric care, primary care, and palliative care.
• Utilizing interventions that maximize health capabilities and reduce unnecessary urgent care and program utilization.
• Collaborating in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
• Demonstrating performance and leadership broad enough to improve care for the HBPC Program and the facility.
• Providing peers with formal or informal constructive feedback for improvement.
• Supporting colleagues and other nurses through HBPC knowledge sharing to promote safe, high-quality nursing care, and foster a supportive environment conducive to professional development.
• Evaluating outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations.
• Maintaining responsibility for documented HBPC outcomes at the program or service level.
• Demonstrating broad, complex HBPC outcomes at applicable organizational levels within the facility.

Pay:
Competitive salary, regular salary increases, potential for performance awards
