The CHOP/SHOP Program

The Community Health Outreach Program (CHOP), originally launched in 2002 as the Senior Health Outreach Program (SHOP) by Dr. Alexander G. Salerno, is the longest-running house-call program of its kind in the area. Built on the tradition of the home visit, CHOP brings fully integrated primary care directly to patients in the places they live and gather-private homes, senior residences, group homes, adult daycare facilities, and behavioral health centers.

Operating under Nirvana's population health office, CHOP delivers proactive, coordinated care to the organization's most at-risk patients-including seniors, homebound individuals, and those managing multiple chronic conditions, mental illness, or substance use disorder. The program's goal is simple and enduring: to make quality, affordable healthcare convenient and accessible, while sparing patients from unnecessary illness and costly hospitalizations.

Position Summary

Nirvana Healthcare is seeking a dedicated Nurse Practitioner to join the CHOP/SHOP team. This is a home-based, field position: the Nurse Practitioner travels to patients in their homes and residential settings to deliver comprehensive primary care. The role is ideal for a clinician who values building relationships with patients and families, working independently in the community, and making a direct impact on an underserved population.

This position is available immediately.

Key Responsibilities
  • Provide comprehensive primary care to patients in their homes and residential settings, including assessment, diagnosis, treatment, and ongoing management.
  • Manage chronic conditions through routine exams, preventive screenings, medication management, and patient monitoring.
  • Develop and adjust individualized care plans in coordination with the patient, family, and the broader Nirvana care team.
  • Deliver patient and caregiver education to support healthy lifestyle habits and informed healthcare decisions.
  • Coordinate care across specialists, pharmacy services, behavioral health, and community resources to ensure continuity.
  • Identify and proactively engage at-risk patients to reduce avoidable hospitalizations and emergency visits.
  • Maintain accurate, timely clinical documentation in accordance with organizational and regulatory standards.
Qualifications
  • Active New Jersey Nurse Practitioner license in good standing.
  • Current national certification as a Nurse Practitioner (FNP, AGNP, or equivalent).
  • Active DEA registration, or eligibility to obtain.
  • Must be bilingual in English and Spanish.
  • Experience in primary care, geriatrics, home-based care, or chronic disease management preferred.
  • Valid New Jersey driver's license and reliable transportation; willingness to travel to patient homes daily.
  • Strong clinical judgment, with the ability to work independently in a community setting.
  • Compassionate, patient-centered approach and a commitment to serving underserved populations.
