Triage Visit Registered Nurse - Home Care Hours
Part‑time, weekend days. Available every other weekend to take calls, triage patients, and make visits as needed. Remote work with optional visits. General hours: 8:00 am – 5:00 pm Saturday and Sunday.
TrainingAvailability during weekdays (Monday‑Friday, 8:00 am‑5:00 pm) for a minimum of 2 days per week, approximately 2‑6 weeks, depending on availability.
Responsibilities- Take calls from the On‑Call Service and address them based on type of calls (e.g., Intake/Referrals, Scheduling, Patient Concern).
- Provide telephone triage and make patient visits, as appropriate.
- Complete accurate and timely documentation of patient/family services provided.
- Provide education and instructions to the patient/family in accordance with the plan of care.
- Administer medications and treatments as prescribed by the physician and in accordance with nursing standards of practice.
- Hourly pay range: $38.46 – $46.86

- Graduate of an accredited school of nursing and licensed to practice as an RN in the state of MN. License is active and in good standing.
- Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years.
- Able to travel within a geographic area serviced by the office. Valid driver’s license, reliable transportation, and proof of auto insurance required.
- Current CPR certification required.
