Registered Nurse – Bronson At Home (Home Health) – PRN

Location:

BAH Bronson at Home

Schedule:

First Shift, Part time, 10 scheduled weekly hours

Responsibilities

The Registered Nurse provides skilled nursing services to home care clients according to a physician prescribed plan of care and Medicare Conditions of Participation. The RN works as part of a geographic or specialty team under the direction of the HH Clinical Services Manager and is responsible for:

  • Utilizing the nursing process of assessment, planning, diagnosing, implementation and evaluation
  • Coordinating care with other professionals and delegating tasks within scope of practice
  • Providing physical care elements appropriate to patient acuity and complexity
  • Documenting patient responses and anticipated needs during each visit
  • Communicating effectively with interdisciplinary team regarding the patient’s plan of care

Additional core nursing activities include:

  • Collecting comprehensive data about the patient’s health situation
  • Assessing physical and developmental needs
  • Analyzing assessment data to determine patient problems
  • Documenting nursing history, age‑appropriate needs and strengths
  • Reviewing vital signs and detecting deviations from normal
  • Clarifying problems with patient and caregiver when appropriate
  • Developing and documenting expected outcomes and plan of care using clinical pathways
  • Implementing prescribed interventions and medications per approved techniques
  • Utilizing evidence‑based interventions specific to the patient’s plan
  • Managing care plan by organizing and delegating interventions, and evaluating effectiveness
  • Initiating patient education plans and coordinating implementation
  • Evaluating progress toward desired outcomes and efficacy of interventions
  • Documenting patient response to interventions

  • Advocate for patients and facilitate informed decision‑making
  • Incorporate research findings into nursing practice and policies
  • Consider safety, effectiveness, cost, and impact when planning services
  • Delegate tasks based on patient needs, harm potential, and complexity
  • Lead within the professional setting and participate in committees and councils
  • Attend at least 50 % of unit staff meetings
  • Create and maintain a professional and healthy work environment as representing Bronson
Qualifications
  • Diploma, Associate’s, or Bachelor’s degree in Nursing (required)
  • Licensed RN in good standing with the State of Michigan (required)
  • 2 years of continuous experience in an acute care setting preferred
  • Current driver’s license in Michigan and valid auto insurance (required)
  • BLS certification (required)
  • Own vehicle for scheduled travel and ability to pass MVR check before start date
