Registered Nurse – Home CareAs a Home Care Registered Nurse, you’ll play a vital role in helping patients receive high‑quality care in the comfort of their own homes. You’ll provide skilled nursing services based on a physician‑ordered plan of care while supporting patients and families every step of the way. In this role, you’ll work as part of a collaborative geographic or specialty care team under the guidance of the Home Health Clinical Services Manager.
Using the nursing process, you’ll assess patient needs, plan and deliver care, and evaluate outcomes to ensure the best possible experience for each patient.
- Deliver skilled nursing care to home care patients according to physician‑prescribed plans of care and Medicare guidelines
- Assess, plan, implement, and evaluate individualized patient care
- Coordinate care with physicians, therapists, and other members of the care team
- Delegate appropriate tasks to patient care assistants and support staff
- Provide hands‑on nursing care based on patient needs and level of complexity
- Document each patient visit, including responses to care and ongoing needs
- Communicate effectively with the interdisciplinary team to support continuity of care
- Demonstrate competency in caring for the patient populations served
- Diploma, Associate’s, or Bachelor’s degree in Nursing required
- Active RN license in good standing with the State of Michigan required
- BLS certification required
- Two (2) years of recent continuous acute care experience preferred
- Valid Michigan driver’s license and proof of auto insurance required
- Successful MVR check required after offer and prior to start date
- Proof of active auto insurance must be maintained and updated with Bronson as needed
- Driving the position requires regular use of a personal vehicle for work‑related duties
- Ability to assess, plan, implement, and evaluate patient care using the nursing process
- Strong communication skills (verbal, written, and electronic)
- Ability to interpret clinical and electronic data to guide care decisions
- Comfortable with sustained computer use and detailed documentation
- Ability to lift up to 50 pounds and assist with patient care activities
- Must be able to constantly communicate both verbally, in writing and electronically
- Read, interpret, and analyze data from various computer systems and equipment in order to set an appropriate plan of action
- Administer prescribed medication and treatments in accordance with nursing standards
- Review vital signs and record patient condition during visits
- Deliver evidence‑based, patient‑centered care
- Document care, patient responses, and outcomes accurately
- Collaborate with patients, families, and interdisciplinary teams
- Participate in quality improvement, education, and professional development
- Provide culturally sensitive, ethical, and compassionate care
- Serve as a patient advocate and contribute to a positive work environment
- The registered nurse systematically evaluates the quality and effectiveness of nursing practice
- Participates in quality improvement activities
- Uses the results of quality improvement activities to initiate changes in nursing practice
- Maintains professional records that provide evidence of competency and continued ongoing educational activities
- Provides culturally sensitive, age‑appropriate care
- Identifies areas of strengths and opportunities for professional development through self‑evaluation
- Contributes to a supportive and healthy work environment
- Collaborates with patient, family, and others in nursing practice
- Protects patient autonomy, dignity, confidentiality, and rights
- Integrates research findings into practice and informs evidence‑based policy development
