You’ll provide comprehensive primary care to senior patients at assisted living facilities within 30 miles of Seattle. You’ll typically work at one facility per day, supported by a dedicated care management team that knows your patients and handles the busywork—so you can focus on clinical judgment and relationships.

Your week: ~3–4 days in the field, ~1–2 days on telemedicine and care coordination (about 30–50% virtual/flexible work).

• Conduct comprehensive assessments and build individualized care plans
• Collaborate with care managers, physicians, and facility teams
• Educate and support patients and families
• Monitor progress and adjust treatment plans
• Travel to facilities within 30 miles of Seattle

• Active WA Nurse Practitioner or Physician Assistant license
• Deep compassion and care for patients; willingness to work closely and collaborate with other members of the team
• Senior care / home health experience preferred (assisted living a big plus)
• Strong primary care and care management knowledge
• Excellent communication and ability to work independently, with excellent clinical judgement

$145,000–165,000 + $5–10,000 sign‑on bonus

CME allowance of 3 days and $3,000 + malpractice coverage
