Role Description

This is a full-time, on-site Private Duty Nurse role based in Anoka, MN, providing in-home care within the surrounding area. Private Duty Nurses will deliver one-on-one skilled nursing care, such as medication administration, wound care, tracheostomy and ventilator management (if applicable), monitoring vital signs, and implementing individualized care plans. Daily responsibilities include conducting thorough assessments, documenting client status, coordinating with physicians and care teams, and promptly responding to changes in condition.

The nurse will assist with activities of daily living as needed, educate clients and families on care routines and health management, and help maintain a safe, clean, and comfortable home environment. The role also involves clear communication with supervisors, participation in care conferences, and adherence to all clinical, ethical, and regulatory standards.

Qualifications
  • Current, active RN or LPN license in the state of Minnesota, in good standing, with eligibility to provide home care services.
  • Demonstrated clinical skills in areas such as medication administration, wound care, chronic disease management, and monitoring of vital signs.
  • Experience with private duty, home health, long-term care, or similar one-on-one patient care settings is strongly preferred.
  • Ability to perform thorough patient assessments, follow individualized care plans, and document care accurately and promptly in accordance with agency policies.
  • Strong interpersonal and communication skills to build trusting relationships with clients, families, and interdisciplinary care teams.
  • Commitment to compassionate, person-centered care that respects client dignity, cultural backgrounds, and individual preferences.
  • Reliable transportation and willingness to travel to client homes within Anoka, Hennepin, Ramsey Counties, and nearby communities as assigned.
  • Capacity to work independently in the home setting, exercise sound clinical judgment, and respond effectively to changing client needs.
  • Basic life support (BLS/CPR) certification required; additional certifications (e.g., ACLS, ventilator or tracheostomy care training) are a plus.
