Nurse Practitioner – Palliative Medicine (Full Time, Bilingual preferred)
Salary: $108k-$135,000 based on years of experience.
We are seeking a diligent and compassionate Nurse Practitioner for our Palliative Medicine Inpatient team. The NP will work with our Palliative Medical Partners group providing consultation and follow-up visits for patients within our community hospital and specialty hospital partners in New Jersey. The NP will collaborate with patients, families, and their primary care and consulting physicians to provide excellent patient and family centered care, while developing and maintaining internal and external relationships to meet the needs of our patients and their families.
The position requires a Master’s degree in nursing, RN registration, and an APN license in New Jersey. Experience in palliative care, oncology, hospice or geriatric care is preferred but not required.
- Provide health assessments appropriate to the visit, diagnose, and treat patients requiring symptom management and goals of care conversations.
- Collaborate with patients, families, attending physicians, and other specialists to deliver patient and family-centered care.
- Develop and maintain internal and external relationships, working with support staff to meet the needs of partners.
- Work alongside Care Navigators to coordinate care across Samaritan's family of programs.
- Provide continuity of care by collaborating with Navigators and Complex Care Coordinators to transfer inpatient patients to the outpatient palliative care team.
- Work full-time, Monday–Friday (40 hours per week) with after-hours call rotation (telephone calls, no visits).
- Participate in on-call rotation and weekend schedule, with compensated days included.
- Preferred experience in palliative care, oncology, heart failure, geriatric care, and/or hospice.
- RN registration and APN license in New Jersey.
- DEA and CDS license required.
- Consultations in partner hospital settings.
