Nurse Practitioner

Carease is a nationwide preventative healthcare company focused on reducing hospitalizations. We are dedicated to improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care. Carease Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities.

This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player. • Active NP or PA license (in the state of practice)
• 1+ year of experience in Geriatric care
• 1+ year of experience in Documentation review
• Excellent communication, collaboration, and teamwork skills
