• Provide coordinated, high‑touch primary care for Medallion patients.
• Partner with physicians and interdisciplinary team to manage acute and chronic needs.
• Serve as key point of contact for patients, ensuring continuity of care.
• Triage patient concerns and coordinate care activities.
• Perform clinical assessments and support medical procedures.
• Educate patients and families, facilitate transitions of care.
• Lead as preceptor or formal leadership, participate in quality improvement.
• Delegate patient care based on skill level, acuity, resources, and fiscal accountability.
• Maintain excellent communication, mentoring, and teaching skills.
• Drive personal vehicle if required by role.

Graduate of a baccalaureate nursing program accredited by ACEN, CCNE, or NLN CNEA. If not accredited, at least one year of RN experience in an applicable care setting is required.

Entry‑level associate degree RN must provide documentation of baccalaureate completion within five years of the RN start date if start after April 1 2020, or as required by the program in place at hire if before that date.

• High‑touch, customer‑service oriented preferred.
• Ambulatory or family medicine/primary care experience preferred.
• Two years of nursing experience within the last five years preferred.
• Excellent verbal and written communication skills required.
• Experience working in a team environment required.
• Computer skills required; prior EMR experience preferred.
• Availability for flexible hours, including evenings, nights, holidays, weekends, and on‑call.
• Ability to adapt to unpredictable situations.
• Demonstrated leadership, effective communication, and critical thinking skills.
• Current RN license in the applicable state.
• Arizona RN must maintain BLS competency unless location exempted.
• Additional state licensure or specialty certification/training may be required.

Full time, 8 : 00 am – 5 : 00 pm, Monday – Friday. 80 hours per pay period. No weekend schedule.
