We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. As an RN, you demonstrate great attention to detail and a caring heart. You take pride in creating personalized care plans and can observe and communicate effectively in your patient’s best interest.

Service area:
Port St. Lucie to Sebastian, FL. Flexible schedule and hybrid work opportunities available.

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
• Oversee and support in‑home caregiver teams
• Ensure compliance with care plans and agency standards
• Document any changes in patient conditions and report on changes
• Maintain accurate records of patient care and discuss observations with the supervisor

• Currently licensed Registered Nurse (RN) in the state of employment
• Previous experience as an RN is preferred
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required
• English required;
  Spanish and Creole speakers a plus
• Reliable transportation
• Ability to pass a Level 2 Background Check and I-9 verification
