Hospice Admissions Registered Nurse (RN)

We are partnering with a well-established, multi-state hospice provider known for delivering compassionate, patient-centered care through interdisciplinary teams in both home and facility settings. We're seeking a dedicated Hospice Admissions Registered Nurse (RN) to join our team for a full-time position (40 hours per week).
Pay and Schedule
$40–$45/hour
Monday–Friday, 10:00am–6:00pm
- Some back-up on-call rotation
- Full-time, 40-hour work week
- Structured onboarding and training program
- Includes weekend coverage (Saturday and Sunday)
- Includes holiday participation (minimum one summer and one winter holiday)
- $6/hour hold rate plus time-and-a-half for visits during call

Responsibilities
- Conduct hospice admission assessments in homes, assisted living facilities, skilled nursing facilities, and other care settings
- Evaluate patient eligibility for hospice services in collaboration with physicians and the interdisciplinary team
- Educate patients and families on hospice philosophy, services, and benefits
- Complete admission documentation and coordinate the transition to hospice care
- Develop initial plans of care and facilitate a seamless handoff to the RN Case Manager
- Collaborate with physicians, social workers, chaplains, aides, and clinical leadership
- Maintain timely and accurate documentation using Homecare Homebase
- Provide compassionate education and emotional support to patients and caregivers during the admission process
Territory
Coverage includes:
- Cumberland County
- Perry County
- Franklin County
- Adams County
Patients are assigned geographically to promote efficient scheduling and travel.
This role includes a mix of home-based and facility-based hospice admissions throughout the service area.

Requirements
- Active RN license
- At least one year of hospice experience
- Strong assessment and communication skills
- Ability to educate patients and families on hospice services
- Valid driver's license and reliable transportation
