As a Hospice CNA/Home Health Aide, you will play a vital role in supporting patients and their families during some of life's most intimate and challenging moments. At every visit, you will help create a nurturing, respectful, and comforting environment, ensuring patients feel valued, cared for, and supported.

• Provide hands‑on assistance with daily living activities such as bathing, grooming, dressing, toileting, and mobility.
• Monitor and report changes in patient condition to the hospice nurse or care team, ensuring timely interventions.
• Deliver emotional, spiritual, and compassionate support to patients and their families, fostering a sense of comfort and understanding.
• Help families navigate the hospice process, offering guidance and reassurance during difficult decisions and transitions.
• Maintain a safe, clean, and organized environment for patients, promoting comfort and dignity at all times.
• Document care provided accurately and timely and communicate effectively with the interdisciplinary team.

• Compassion, empathy, and patience in caring for patients and their families.
• Strong communication skills to collaborate with patients, families, and the care team.
• Reliability and flexibility to meet the needs of patients in a dynamic care environment.
• Experience as a CNA, HHA, or similar role is preferred, but a heartfelt dedication to caring for others is essential.
• Ability to provide comfort, respect, and advocacy for patients in end‑of‑life or home health care.

• The job requires driving to where the patient lives which could be a nursing home, an assisted living facility, or a private residence
• The main home office is in Marlborough, MA
• Must have a reliable vehicle
• Per Diem / as needed
• There is some flexibility on the hours
• Rotating holiday and limited weekend schedule

• Assist patients with their daily activities
• Care for patient's personal needs
• Provide thorough care

• Minimum of one year of experience as a Certified Nursing Assistant
• Hospice experience preferred

• Must be able to read and write in English and follow written and verbal instructions in English
• Demonstrates interest in the welfare of ill and elderly
• Must pass Aide Competency Evaluation Skills
