Location: Roseville, CA
- Hybrid
- Remote (CKD) This is a hybrid/remote, field based role requiring a dedicated home office and the ability to travel to meet business and patients needs, including team and nephrology partner meetings. Though this position is primarily remote, it may require the RN Case Manager to be in the field 1-2 days per week in an assigned territory. Coverage includes Roseville, Auburn, Grass Valley and surrounding areas.

Territory assignments may change based on business needs.

Employment Type: Full-Time

This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients. The RN Case Manager coordinates care between the patient, family, physician, provider, and care teams; engages the community; and collaborates with the Regional Operations Manager.

About the Employer DaVita Integrated Kidney Care (DaVita IKC) is the integrated care division of DaVita Inc., working on DaVita’s vision to provide integrated care to all ESRD patients.

Responsibilities

  • Coordinate and facilitate care for ESRD and CKD patients across multiple providers and settings.
  • Develop individualized care plans and monitor progress.
  • Engage patients and families in the care process.
  • Collaborate with nephrology partners, community resources, and the Regional Operations Manager.
  • Assess patient needs and coordinate services across interdisciplinary teams.
  • Provide support during complex healthcare transitions.

Qualifications

  • A.D.N degree from an accredited school of nursing required; B.S.N preferred; a three‑year diploma from an accredited diploma program may substitute for a nursing degree.
  • Current Registered Nurse (RN) license in the state of practice required.
  • Continuing education credits maintained as required by the state of practice required.
  • Minimum of five (5) years’ experience in clinical nursing required.
  • Minimum of three (3) years’ experience in renal nursing preferred.
  • Case management experience required; alternatively, 3+ years of experience assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex transitions.
  • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred.
  • Demonstrated knowledge of clinical, financial, and patient satisfaction outcomes.
  • Experience as a change agent and in continuous quality improvement techniques.
  • Previous experience in healthcare performance coaching required.
  • Current CPR certification required.
  • Ability to adapt to new electronic processes and increased productivity expectations (e.g., Capella implementation).
  • Current driver’s license with a positive driving record and able to meet insurance coverage requirements.
  • Basic computer skills and proficiency in MS Word and Outlook required.
  • Functional proficiency with DaVita specific clinical software programs, including Capella, required within 90 days of employment.
  • Home office with internet connectivity at minimum of 1MB upload and 1MB download speed required.

Salary Range $87,400–$114,000 per year. Compensation will depend on qualifications, skills, competencies, and experience.
