Location Stuart to Vero Beach, FL Hybrid
Schedule Monday thru Friday, 8AM to 5PM
Salary $72,000 annually plus Benefits

Lead and mentor home health staff and aides
Oversee patient care coordination and program operations
Ensure clinical excellence and compliance with state and federal standards
Collaborate with referral partners, physicians, and interdisciplinary teams
Enjoy a consistent Monday to Friday schedule and supportive team environment
Key ResponsibilitiesManage patient intake, authorizations, and care planning for skilled and nonskilled services
Conduct start of care and supervisory visits, as well as orientations and training for staff
Provide clinical oversight aligned with best practices and agency policies
Support and mentor field clinicians to ensure quality care delivery
Maintain compliance with all regulatory and accreditation requirements
What You’ll BringActive RN license in Florida
, required
Minimum 1 year of home care experience
, preferred
Experience with
Hospital at Home programs
, highly preferred
Valid driver’s license and reliable transportation
Strong communication, leadership, and time management skills
EMR proficiency;
Alaya Care experience a plus
