Hospice RN Case Manager

Overview

Sentara Health is seeking to hire a qualified individual to join our team as a Direct Care Registered Nurse - Home Health and Hospice. This role provides high-quality nursing care to patients in their homes or hospice settings, ensuring comfort, dignity, and emotional support during end‑of‑life care.

First (Day) Shift, Full Time (40 hours/week)

Location:

Sentara Hospice Services, Chesapeake, VA

Pay Range: $33.61–$56/hour (market‑competitive compensation packages, including base pay, incentives, and benefits)

• Associate's Degree in Nursing (Required)

• Valid RN License in state of practice (VA) (Required)
• BLS (Required within 90 days of hire)

• 1 year of nursing experience (Required)
• Previous Home Care/Hospice experience (Preferred)
• Prior case management experience (Preferred)
