Family Nurse Practitioner - Part time - Evernorth - Kokomo

Position Summary

• Provide primary care treatment in a workplace-based setting.
• Deliver episodic care (low acuity urgent care).
• Provide extended episodic acute care (up to 4–5 visits) for illnesses or injuries that are limited in chronicity.
• Educate patients about chronic conditions and co‑manage with outside primary care providers if EWC is not the PCP.
• Oversee and perform CLIA‑waived tests, dispense prescription medications, and manage a specimen collection lab.
• Collaborate with onsite employees to provide biometric screening and health and wellness education.
• Potentially perform travel medicine recommendations and immunizations or administration of allergen immunotherapy (with proper training).
• Deliver one‑on‑one health coaching to employees and refer patients into wellness programs.
• Partner with local HR/Benefits teams to improve employee health.
• Work closely with or potentially oversee members of the health care team (MA, LPN, etc.).

• Graduate of a certified NP program.
• Active and unrestricted NP license in the respective state, with ability to maintain it.
• Minimum of 2+ years of NP experience, working independently in an internal medicine or family practice setting.
• Ability to practice independently in the respective state; may be asked to obtain licensure in additional states.
• DEA licensure, prescriptive authority, and ability to maintain it.
• Strong primary care experience, including women’s health.
• Chronic disease management experience preferred.
• Electronic Health Record (EPIC) experience is a plus.
• BLS certification and ability to maintain current certification.
• Phlebotomy experience is a plus.

• Excellent interpersonal skills with internal and external customers and within group settings.
• Passionate about overall health and wellness and patient education.
• High energy and strong customer‑centric focus.
• Ability to work clinic hours; solid attendance record.
• Strong time‑management skills.
• Additional language skills such as Spanish are a plus.
• Supervisory experience (indirect or direct) preferred.

This is a part‑time 28 hour/week role based in the health center in Kokomo, IN.
