Overview
Case Management Nurse - Compact RN License. 100% remote, contract to hire potentially, 6+ months. Base pay range: $37.00/hr - $39.00/hr duration: 6+ months with potential for contract-to-hire. Location requirement: must have Compact RN License and reside in one of the following states: CO, GA, IN, KY, MS, OH, PA, VA, WA, LA or IA.
Paid 8-week training. All equipment provided. OpTech has multiple job openings with a Tier 1 client.

Responsibilities- Lead the coordination of a regionally aligned, multidisciplinary team to provide holistic care to meet member needs telephonic and/or digital.
- Coordinate care with Medical and Behavioral Health Social Workers, Registered Dietitians, Pharmacists, Clinical Support Staff and Medical Directors.
- Use the case management process to assess, develop, implement, monitor, and evaluate care plans designed to optimize the member’s health across the care continuum.
- Assess the member’s health, psychosocial needs, cultural preferences, and support systems.
- Engage the member and/or caregiver to develop an individualized plan of care, address barriers, identify gaps in care, and promote improved health outcomes.
- Arrange resources (e.g., community resources, mental health services, substance abuse services, financial support, disease-specific services).
- Coordinate care delivery and support among member support systems, including providers, community-based agencies, and family.
- Advocate for members and promote self-advocacy.
- Deliver education on health literacy, self-management, medication plans, and nutrition.
- Monitor and evaluate the effectiveness of the care management plan; assess adherence and adjust as necessary.
- Document interactions to support member management.
- Prepare members for discharge or transfer to ensure continuity of care; educate on post-transition care and follow-up.
- Coordinate durable medical equipment and transportation services as needed.
- Adhere to professional standards and meet quality and production goals.
- Maintain ongoing professional development and CCM certification.
- Nursing Diploma or Associate degree in nursing required;
Bachelor’s degree strongly preferred. - 3 years of clinical nursing experience in clinical, acute/post-acute, and community settings.
- 1 year of case management experience in a managed care setting preferred.
- Experience managing patients telephonically and via digital channels (mobile apps and messaging) preferred.
- Certification in Case Management (CCM) required or to be obtained within 18 months of hire; CCP certification preferred.
- Compact RN License required.
- Compact RN License (required).
- Certification in Case Management (CCM) required or to be obtained within 18 months of hire.
- Certification in Chronic Care Professional (CCP) preferred.
- Critical thinking, decisiveness, and problem-solving across diverse topics affecting outcomes.
- Empathetic, supportive, good listener; proficient in motivational interviewing.
- Time management and strong organizational skills; ability to manage multiple systems/tools while engaging with a member.
- Proficient in Microsoft Office; strong written and verbal communication; able to work in teams and independently.
- Nursing degree required;
Bachelor’s preferred. - 3 years of clinical nursing experience; 1 year of case management in managed care preferred.
- Telephonic/digital patient management experience preferred.
