Please note that although this is a full‑time telehealth position, candidates must hold an active, unrestricted New York State clinical license as a Nurse Practitioner (NP) or Physician Assistant (PA).

• Annual compensation between $115,000 – $130,000

• Clinical Care Delivery & Care Coordination
  • Deliver comprehensive virtual care, including patient assessments, condition management, medication review, and appropriate escalation, while ensuring compliance with telehealth regulations and licensure requirements.
  • Collaborate with multidisciplinary teams and utilize telehealth workflows to support coordinated, continuous care and maintain accurate documentation of patient encounters.
• Educate patients on preventive care, disease management, and healthy lifestyle behaviors to improve outcomes and encourage active participation in their health.
• Provide compassionate, culturally sensitive care while maintaining professionalism, strong communication, and sound clinical judgment in all interactions.
• Quality & Compliance
  • Adhere to clinical guidelines, regulatory standards, and organizational policies while participating in quality improvement initiatives.
  • Ensure accurate, timely documentation, uphold patient privacy, and maintain high standards of safety, ethics, and confidentiality.

• Active, unrestricted current NY licensure as a Provider in the state.
• Minimum of 3600 hours of practice experience, or 3 years of clinical experience in a primary care or longitudinal care setting (NY State only).
• Board Certification as a Family Nurse Practitioner, Adult‑Gerontology Nurse Practitioner, or Physician Assistant.
• Obtain autonomous provider status as required by state you are licensed with within six months of meeting eligibility requirements.
• Active DEA license.
• Current Basic Life Support Certification.
• Proficiency in electronic medical record (EMR) systems.
• Ability to pass background check upon hire and throughout employment.
