Looking for an amazing Mid‑Level Practitioner (PA or NP) to join our team! We are seeking a skilled and compassionate provider to join our healthcare team. As a full‑time Wound and Primary Care Provider, you will be responsible for providing specialized care to patients with primary care needs as well as wounds, ensuring their comfort and promoting healing. • Perform comprehensive assessments of patients with and without wounds, including diagnostic evaluations
• Develop and implement individualized wound and primary care plans based on assessment findings
• Provide wound care treatments, including dressing changes, debridement, and other procedures at patient’s bedside
• Provide primary care services to patients in an assisted living setting
• Monitor and evaluate the progress of wound healing
• Collaborate with interdisciplinary healthcare team members to coordinate patient care
• Educate patients and their families on wound care techniques and prevention strategies
• Maintain accurate and up‑to‑date documentation of patient assessments, interventions, and outcomes
• Round in long‑term care facilities, assisted living communities, in‑office and occasionally, home visits

• Must hold an active Physician Assistant or Nurse Practitioner License in Idaho
• Experience of 1‑3 years as a provider is preferred but the right new graduate will be trained
• Salary is dependent on experience
• Strong knowledge and experience in wound care management preferred but not required
• Ability to triage patients effectively in emergency situations
• Strong communication skills
• Experience in post‑acute care settings (Assisted Living or Skilled Nursing) recommended but not required
• Driving is required, a dependable vehicle and a valid driver’s license is a must. Mileage is reimbursed
