About the Role

Chronic Care Coordinator (Remote – Michigan) – 100% remote, not a compact state. Join our mission to transform healthcare delivery from reactive, episodic care to proactively managed patient care that prevents life‑changing problems before they happen for patients with two or more chronic conditions. The position is an independent 1099 contractor with the potential to transition to full‑time W2 employment with benefits.

You will perform telephonic encounters each month, develop detailed care plans within the electronic health record, and set your own hours, averaging 20–30 hours per week.

• Perform telephonic encounters with patients on behalf of our partners each month.
• Develop, document, and update detailed care plans in the EHR care‑plan templates.
• Carry a patient panel of at least 100 patients per calendar month within the first three months of assignment.
• Complete encounters on 90% of assigned patients each month, unless patients cannot participate due to health conditions.
• Attend regularly scheduled meetings (e.g., bi‑monthly staff meetings, monthly one‑on‑ones).
• Maintain HIPAA‑compliant work environment and ensure privacy of patient information.

Productivity‑based pay structure:

• $10.00 per completed patient encounter up to 99 encounters/month
• $10.25 for 100–149 encounters/month
• $12.00 for 150–199 encounters/month
• $14.00 for 200–249 encounters/month
• $16.00 for >250 encounters/month
• Incentive compensation of $1 per encounter for bilingual nurses (equal to $3/hr).

• Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
• Current active license to practice as an RN/LVN/LPN in the state of Michigan (non‑compact state) with no disciplinary actions.
• Minimum of two (2) years of clinical experience in Med/Surg, case management, and/or home health care.
• Hands‑on experience with Electronic Medical Records and Windows desktop applications (Microsoft Office
  365, Teams, Excel, etc.).
• Strong initiative, judgment, organization, time‑management, problem‑solving, and decision‑making skills.
• Proficient computer skills; high‑speed Internet; a desktop or laptop running Windows or Mac OS.
• Excellent verbal, written, and listening skills.

• Self‑directed with strong prioritization.
• Demonstrated time‑management and exemplary phone etiquette.

• Ability to troubleshoot minor technological issues related to remote working.
