Home Care Coordinator
Welbe Health PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community‑based services. The Home Care Coordinator plays a vital role by conducting in‑home care assessments, setting the framework for our home care team to help participants thrive, and reporting to the Home Care Manager.
Responsibilities- Coordinate incoming calls related to participants, physicians, and agency services regarding physician orders, participant questions, and referrals.
- Communicate with participants by phone and coordinate effectively with nursing, therapy, aides, social services, and physicians regarding schedule changes, test results, and other related updates.
- Collaborate with Home Care Services staff to track and monitor home care delivery and scheduling hours.
- Partner with the Marketing team to support the enrollment of prospective participants into the program.
- Assist with staffing and scheduling activities, soliciting, and providing manager input.
- Participate in end‑of‑life care, coordination, and support.
- Healthcare/medical licensure or equivalency, with at least three years of professional experience.
- Minimum of three years of case management or nursing experience in a clinical or home setting with a frail or elderly population.
- Nursing knowledge and training necessary to treat frail, elderly participants and care for complicated clinical conditions preferred.
- Bachelor’s degree preferred.

Base salary of $67,829—$89,535 USD plus bonus.
