About the Role
As an LPN/LVN Healthcare Advocate with Solace, you'll work with Medicare patients throughout their healthcare journey. In the LPN/LVN Advocate role, you will guide patients through difficult and complex health concerns and help them achieve their health and wellness goals while also addressing their Social Determinants of Health (SDOH). You’ll be an empathetic listener and an action-oriented guide who will work to actually resolve patient problems.
This is a 1099 contract position. You will set your own hours and can work part-time, full-time, or anything in between. The role is 100% remote with all patient interactions performed via video or phone call.
- Learn Solace's proprietary system, tools, technology, and expectations, while also providing your unique experience in every interaction.
- Build strong, trusting relationships with patients, where listening and empathy are the foundation for every interaction.
- Identify and prioritize patients’ needs and build out a care plan to ensure consistent action over time.
- Develop comprehensive patient care plans that holistically address social determinants of health, i.e. food resources, transportation access, and support at home.
- LPN/LVN license in good standing.
- Experience working with diverse patient populations.
