• Assists with eating, bathing, toileting, personal hygiene, oral hygiene, dressing, grooming, communication, and feeding
• Transport and attends activities or programs with residents
• Complete various daily, monthly, and other required documentation
• Observe & report any changes in symptoms, behaviors, accidents, incidents or injuries to charge nurse
• Perform light housekeeping duties in the living areas
• Apply safety & infection control measures
• Assists with redirecting unsafe actions to bring about positive outcomes
• Assist nursing personnel with admission, discharge & transfer of residents

• Formal education in arithmetic that includes addition & subtraction and in reading, writing & speaking common English vocabulary
• For positions in the Department of Developmental Disabilities, must possess a high school diploma or certificate of high school equivalence pursuant to Ohio Administrative Code 5123:2-3-01(F) (1) (c)
• For positions at Veteran Services in addition to the above must possess STNA certification &/or listed on the Ohio Nurse Aide Registry
• For positions that require operation of a motor vehicle to transport consumers to recreational, educational &/or occupational activities, a valid driver’s license is required
• Equivalent to Minimum Class Qualifications for Employment noted above

May be required to stand for extended periods of time, lift heavy objects from time to time, be exposed to a variety of bodily fluids and infectious diseases from residents, work weekends and mandatory overtime.
