Advanced Practice Provider (APRN/PA) Primary Care-BH Grants (260002K7)

• Provide high quality, patient-centered, comprehensive primary care services in a collaborative outpatient setting
• Diagnose and treat acute and chronic conditions, including ordering and interpreting diagnostic tests
• Develop and implement personalized treatment plans
• Educate patients and their families on disease prevention, health maintenance, and lifestyle changes
• Collaborate with physicians, specialists, and other healthcare team members to ensure high-quality care
• Request, interpret, and evaluate diagnostic and laboratory tests and take appropriate actions as medically indicated in accordance with departmental protocols
• Identify and report any specific signs and symptoms, problems, or potential problems requiring further assessment, communication, and/or intervention to promote positive outcomes
• Work autonomously as well as in collaboration with a variety of individuals to diagnose and manage clients' health care problems

• Demonstrate current knowledge and level of expertise utilizing evidence and theory‑based practice, infection control standards, and clinical evaluations in providing care to clients in clinic or hospital‑based settings, as evidenced by following established protocols
• Proficiency in Microsoft Office suite and electronic medical records (EMR) systems, preferably Epic
• Ability to multitask in a dynamic environment
• Strong written and oral communication skills
• Outstanding customer service and conflict resolution abilities

Current active licensure as Advanced Registered Nurse Practitioner or Physician Assistant in the State of Florida. Family medicine training preferred. Primary care experience in a physician office strongly preferred.
