Position: Registered Nurse (RN) Admissions, Weekends

Schedule

2 weekdays, and every Saturday;
On-Call once per month; 2 holidays per year.

As a Registered Nurse (RN) the work you do every day makes a difference in the lives of our patients. You will provide one-on-one patient care, work at the top of your license, lead care coordination, and deliver home-based nursing care under an attending physician’s direction. You will be part of an interdisciplinary team focusing on compassionate quality care and positive outcomes.

How You’ll Work

You will provide skilled nursing care in a patient’s home as directed by the attending physician. You will work autonomously, managing your time efficiently and following state scope of practice.

Major Areas of Responsibility
  • Plan of Care: observe and monitor patient conditions, perform OASIS assessments, develop individualized care plans, and adjust as needed.
  • Minimize Patient Risk: administer medication as prescribed, prioritize visits for high-risk patients to reduce rehospitalizations.
  • Continuity of Care: manage multidisciplinary care, promote continuity through appropriate admissions, transfers, and discharges.
  • Family Educator/Advocate: educate patient and family on disease processes using teach-back methods.
  • Policies: complete all clinical documentation per agency protocol and Medicare/Federal guidelines.
  • Collaborate with the interdisciplinary team and physicians to ensure optimal care.
  • Rules and Regulations: follow agency policies, procedures, and regulations and communicate schedule changes.
  • Operations: attend in-service training and mandatory agency meetings.
Requirements
  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute-care setting (home-care experience preferred).
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
