Advanced Practitioners - Other Specialties

• Per diem Nurse Practitioner or Physician Assistant opportunity within the REACH (Resource for Elderly And Care at Home) Program serving geriatric patients in the greater Bridgeport area
• Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA)
• Home care-based position working within a multi-disciplinary care team in a geriatric medicine setting
• Per diem schedule, Monday through Friday weekdays
• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients under established guidelines
• Develop plans for further evaluation of health needs, including referrals to other health team members or specialists in consultation with physicians and care team members
• Provide appropriate treatment for problems within scope of practice and identify and refer issues outside scope to the appropriate physician
• Unrestricted State of Connecticut Medical License required (or eligible)
• APRN (Family, Adult/Geriatric Nurse Practitioner) or PA training required
• Minimum of two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA) required
